Requirements: Required Certifications: BLS, LCSW, LMSW Required Skills: Short Term Acute Care Inpatient Medical Social Worker, Assess response to treatment, Biopsychosocial assessment, Clock Drawing Dementia Screen, Develop plan of care to meet patient/caregiver needs, Coordinate services to home care and equipment following discharge, Documents pertinent information to facilitate care coordination, Participates in care conference activities, Refers to rehab and long term care facilities, Supervises staff effectively and efficiently, Discharge Summary, Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response, Facilitate discharge and contingency planning, Educates patient/caregiver to facilitate discharge, Provide / reinforce individualized patient/caregiver education, Arrange transfers to physician appointments, and discharge home, Complete all paperwork and accountability reports, Discharge Planning Job Details: Pay: $1927/week - $2153/week Assignment Length: 13-week assignment Shift: 5, 8-Hour 08:00 - 16:30 At least one year of experience required Aya delivers: The most jobs in the industry.
